Jay Anthony Polack, age 59, passed away peacefully on Thursday June 12, 2025. He was born on October 8, 1965 to the late Jack and Kathy (Guy) Polack.
Jay grew up in Caledonia and was a graduate of UW Oshkosh. Jay proudly served his country for 4 years as a member of the US Air Force. He was a very active parent and spent several years coaching his sons’ hockey teams as they grew up. He was an avid Packers fan and loved watching and cheering them on. In his earlier years, he enjoyed hunting, fishing, and the occasional round of golf.
Jay is survived by his children: Logan (Kat) Polack, Lashaunda (Cody) Warlick, and step-son, Justin (Erin) Tanksley; grandchildren: Charli and Lennon Polack; Addie, Gianna, Ben, and Jaxson Warlick; and Savannah, Beighley, and Aubreigh Tanksley. He is further survived by his siblings: John, Jeff, and Jamie; along with many extended family members and friends.
He was preceded in death by his parents: Jack and Kathy.
A Memorial Service for Jay will be held on Wednesday, June 18, 2025 at 11:00 a.m. at Verkuilen-Van Deurzen Family Funeral Home, 101 Canal St., Little Chute.
